Visiting Nurse and Community Care (VNCC) provides a wide range of comprehensive professional healthcare services with the goal of enabling clients to lead healthy, productive lives while remaining in the comfort and convenience of their own homes.
Because no two cases are exactly alike, it is necessary to develop a customized plan of care for each client. We work closely with the primary care physician, a multi-disciplinary medical team and the family, offering specialized, individual care and clinical consistency.
Each client is assigned to a Care Manager, who is responsible for planning, coordinating and overseeing all services.
VNCC services fall into three main categories:
When there is a need for ongoing medical care, VNCC provides skilled nursing care in the home in conjunction with the primary care physician and our multi-disciplinary team. This includes administering medication while monitoring and promoting a healthy lifestyle regimen. In addition, we offer rehabilitation services, home care assistance and counseling by a licensed social worker.
Other specialized services – such as wound ostomy nursing care, cardiovascular treatment and diabetes management – are also available.

VNCC Personal Care provides individualized assistance with daily living activities, such as meal preparation, light housekeeping, personal hygiene, dressing, errands and laundry. We also provide medication reminders and transportation to medical appointments.
Personal Care enables loved ones to age gracefully in their own homes and relieves the stress many families experience when they feel they’re “on call” 24 hours a day.

Focusing on the client and the family, VNCC provides hospice care for loved ones who are chronically or terminally ill, allowing them to live out their remaining days at home, in comfortable surroundings among family and friends.
We provide emotional support and assistance as well as state-of-the-art palliative (pain management) care.
