We offer skilled nursing care in the home in conjunction with the primary care physician and our multi-disciplinary team. This includes monitoring and promoting a healthy lifestyle regimen as well as providing rehabilitation services.
These services enable loved ones to age gracefully in their own homes by providing meal preparation, light housekeeping and medication reminders as well as help with personal hygiene, dressing, errands, shopping and transportation to medical appointments.
Focusing on the client and the family, we enable you to care for your loved ones at home, providing support and assistance as needed. We also help the family access important information and resources during this challenging time.
In March 2017, Greater Medford Visiting Nurse Association and Visiting Nurse and Community Health merged to form Visiting Nurse & Community Care.
Both of these nonprofit organizations had been providing home health care and community health programs for more than a century and will continue to offer the same high-quality services to 30+ communities in the greater Medford/Arlington area.

VNCC is certified by the Commonwealth of Massachusetts to accept Medicare and Medicaid. We also care for HMO (managed care) and private insurance clients, in addition to those who choose to pay privately.
VNCC has deep roots in the community and is one of the few independent visiting nurse associations remaining in Massachusetts. This independent culture allows us to remain true to our mission of health promotion and disease prevention, and to work effectively with healthcare professionals and the clients they serve.
