Health and Wellness Program

Visiting Nurse and Community Health’s Health and Wellness program extends the quality of care accorded to all our patients to include postdischarge monitoring, community outreach and education. VNCH is dedicated to maintaining the well being of our patients and the communities we serve by providing proactive outreach programs.

Our Health and Wellness Nurse follows up with discharged patients during the critical days just after discharge to ensure their well being and overall safety. VNCH’s extended outreach to our communities includes regularly scheduled Blood Pressure Clinics at various community locations, Immunization Clinics, Educational Programs, and participation at numerous Health and Wellness Fairs throughout the year.
