Skilled Nursing

Visiting Nurse and Community Health’s Registered Nurses are fully qualified to provide a wide range of comprehensive, professional skilled nursing care that allows our clients to stay in the comfort of their home. Working closely with the primary care physician and multi-disciplinary team, a lead nurse acts as the patient’s case manager, while providing the necessary education regarding the patient’s medical regimen.

Our services include, but are not limited to, nursing care in medication management, infusion therapy, dressing and catheter changes, rehabilitation therapies, mental health, and end-of-life care.

VNCH provides services to patients whose medical and psychological conditions no longer qualify for an acute facility stay, but still require the expertise and knowledge of a skilled nurse to ensure their health, well being and safety.
