of a patient in his/her
home environment. Our
social work support
program provides home
services and community
outreach via a team of
professionals who
collaborate to formulate
a plan of care, and
provide ongoing
interventions, education
and support.
Our
services include
counseling for patients,
families and caregivers;
treatment of depression,
anxiety, abuse and
neglect; crisis
intervention; and
assessment of and access
to community resources
(e.g., adult day care,
finances).
VNCH
provides services to
patients needing
long-term care,
including linkage,
support and referral to
other sources; patients,
families and caregivers
having emotional
difficulties coping with
existing medical
conditions; patients
with a diagnosed organic
mental disorder (i.e.,
Alzheimer’s); patients
who suffer cognitive
impairment secondary to
an underlying illness
(i.e., depression,
anxiety); and patients
emotionally unable to
leave home and/or access
services.
