Due to continued rapid growth, Visiting Nurse and Community Health has the following opportunities:

OCCUPATIONAL THERAPIST:
Full Time and Per Diem. Provide OT services to patients in their homes, updating rehabilitation care plans regularly, assessing goals and recommending termination of services when appropriate. Minimum of two years experience in an acute care or rehabilitation facility and current professional Occupational Therapy licensure in the state of MA required.


COMMUNITY RNs:
Per diem. Responsible for assessing the physical, educational and psychological needs of the patient/family following established procedures and providing quality care. Strong assessment skills needed. Current RN licensure in MA required.


ENTEROSTOMAL NURSE:
Per diem. The ET Nurse will provide consultations to nursing staff on the treatment of complex draining wounds, non-healing pressure sores, incontinence and fistulas as well as provide education to the patient and family in the proper use and care of equipment and supplies. Current Massachusetts RN licensure and board certification in enterostomal therapy and three to five years experience required.


HOSPICE NURSES:
On-call positions (weekday evenings, and weekends). Work as part of the multidisciplinary team to provide assessment, planning, and care to the patient and their family members in the home. Current RN licensure in MA, and a minimum of one to two years experience in med/surgical, oncology, home health, or hospice required. Must possess strong interpersonal skills and the ability to provide effective support to the patient and family.


PHYSICAL THERAPIST:
Part time, per diem, weekends. Develop and implement a plan of care prioritizing needs appropriately and create, jointly with patient, family, and other team members, individual teaching plans that will achieve optimal level of patient independence. Home care experience preferred. Current professional licensure in MA required.


HOMECARE SOCIAL WORKER:
Per diem. Provide clinical and non-clinical social work services to patients and families in the home. Current LICSW licensure in the state of MA. Minimum 1-2 years as a social worker in acute medical or related health care setting. Preferred 2-3 years of knowledge of and experience in home care, familiarity with community resources, and elder care experience.


HOSPICE/HOME HEALTH AIDES:
Per diem. Provide personal care, assistance with transfers, ambulation, exercise programs, and related household tasks. Home Health Aide Certificate required.
