Home Health Nurse
Full time and part time, benefited.
Responsible for assessing the physical,
educational and psychological needs of
the patient/family following established
procedures, and providing quality care.
Strong assessment skills needed. Current
professional licensure in MA.
Liaison Nurse
Per Diem. Assists in the facilitating of referrals to home care agencies for patients in need of services. Coordinates ancillary services as needed. Serve as a backup to Intake Nurse. Two years of nursing experience required.
Weekend Nurses
Part time. Provide quality care for a diverse patient population. Weekend incentive pay available. Strong assessment skills needed. Current professional licensure in MA.
Per Diem RN's
Day/evening/weekend shifts. Provide
quality care for a diverse patient
population. Strong assessment skills
needed. Current professional licensure
in MA.
Physical Therapists
Full time, part time or per diem.
Home care experience preferred. Current
professional licensure in MA.
Social Workers (LICSW)
Per diem. Home care experience preferred.
Current LICSW professional licensure in
MA.
Home Health Aide:
Per diem. Provide personal care, assistance with transfers, ambulation, exercise programs, and related household tasks. Home Health Aide Certification required.
Hospice Manager
We are currently seeking a Hospice Manager to play a key role in the successful implementation of a new hospice program. The position requires a high commitment, creativity, and the ability to work in a results-oriented team environment.
The Hospice Manager is responsible for coordinating and managing services provided by an interdisciplinary team to hospice patients and their families, participating in program development and changes, monitoring agency compliance with state health department/federal government standards and regulations, and participating in hiring, orientation and performance management of the patient care team.
Qualifications include a BSN, master's preferred, with current RN licensure in MA, minimum of three years management and clinical experience in hospice, home care or related program, and strong communication, interpersonal, and critical assessment skills.
Hospice Nurses
Full time, part time, and per diem. (weekdays, evenings, and weekends) as well as on-call positions.
Current RN licensure in Massachusetts and a minimum of one to two years experience in med/surgical, oncology, home health, or hospice required.
Hospice Social Worker
Full Time or Part Time. Participate as a member of the interdisciplinary hospice team providing direct case work and counseling services to patients and their families in the home. Minimum 1 to 2 years experience in acute medical or related health care setting. Bereavement or counseling experience necessary. Current MSW licensure in MA required; LCSW or LICSW preferred.
