Due to continued rapid growth, Visiting Nurse and Community Health has the following opportunities in home care and hospice:


HOSPICE NURSES:
Part time, and per diem. (weekdays, evenings, and weekends) as well as on-call positions. (overnight and/or every other weekend) Work as part of the multidisciplinary team to provide assessment, planning, and care to the patient and their family members in the home. Current RN licensure in Massachusetts and a minimum of one to two years experience in med/surgical, oncology, home health, or hospice required.


INTERNAL RN CASE MANAGER:
Full time. Ensure appropriate delivery of quality patient care, proper utilization of nursing services and coordination of home health services. Current RN licensure in MA, clinical and home care case management experience required.


HOSPICE/Home Health AIDES:
Per diem. Provide personal care, assistance with transfers, ambulation, exercise programs, and related household tasks. Home Health Aide Certification required.


PHYSICAL THERAPIST:
Part time. Develop and implement a plan of care prioritizing needs appropriately and create jointly with patient, family, and other team members, individual teaching plans that will achieve optimal level of patient independence. Home care experience preferred. Current professional licensure in MA required.


OCCUPATIONAL THERAPIST:
Part Time. Provide OT services to patients in their home, updating rehabilitation care plans regularly, assessing goals and recommending termination of services when appropriate. Minimum of two years experience in an acute care or rehabilitation facility and current professional Occupational Therapy licensure in the state of MA required.


QUALITY IMPROVEMENT NURSE:
Part Time, 24 hours. Responsible for collection, evaluation, and dissemination of performance data relative to VNCH quality improvement programs. Performs ICD9 Coding. Homecare experience and ICD-9 coding experience required. Past QI experience preferred.


RN CASE MANAGER:
Full time. Responsible for assessing the physical, educational, and psychological needs of the patient/family following established procedures and providing quality care. Strong assessment skills needed. Curent RN licensure in MA required.


COMMUNITY RN:
Part Time or Per Diem, Day and Evening shift (4:00 pm - 8:00 pm) available. Provide quality care for a diverse patient population. Strong assessment skills needed. Current RN license in MA required.


NETWORK ADMINISTRATOR:
Full Time. This position provides software and hardware support, network administration and troubleshooting in a LAN, MAN, WAN environment. Experience with Windows network operating systems administration as well as advanced knowledge of Windows XP, Server 2003, MS Exchange, Domain user/group security and management, policy management, firewall configuration and auditing is necessary. Must have a strong working knowledge of MS Office products. Experience with SonicWall devices, MS SQL, Horizon Homecare, router configuration, VOIP VLANs and subnetting desired. Degree or accredited certificate in networking or computer science needed.
