subsidiary, Home Care Plus,
provides the continuity of
care and professional
services for patients
needing assistance beyond
what health insurance
programs generally cover.
Health care providers are
available 24 hours a day,
seven days a week.
Our services are
designed specifically so
that our patients
receive the highest
quality care, while
maintaining an
independent, dignified
life in the home. From
Registered Nurses, to
companions and home
health aides, we give
the compassionate home
health care our patients
deserve. In keeping
with this philosophy,
VNCH offers HomMed, a
state-of-the-art
telemedicine device that
is placed in the home
and accurately registers
blood pressure and
oxygen saturation
levels, pulse, weight
and temperature every
day. Results are
transmitted to a central
station at VNCH, and one
of our registered nurses
interprets the readings,
logs them, and calls the
patient’s physician, if
necessary. This
provides great peace of
mind for both the
patient and his/her
family.
VNCH
provides services to
patients requiring
additional support and
social services (e.g.,
beyond what Medicare or
private insurance provides)
in order to function safely
in their home environment.
