Home Health Assistance

Visiting Nurse and Community Health’s certified and fully trained Home Health Aides are a vital component of our Multidisciplinary Care Team, the goal of which is to help each and every patient achieve his/her maximum level of health and independence. Under the supervision of a licensed clinician, our Home Health Aides play a critical role in maximizing patient care. They offer valuable assistance in ensuring a patient’s well being by providing emotional support and continuity of care.

Our services include assisting with activities of daily living (e.g., bathing, dressing, oral hygiene, hair and skin care, meal preparation and feeding); assisting with prescribed exercise and therapy activities, ambulation, transfers, and specialized equipment; performing simple dressing changes; and communicating with team members on reported changes in a patient’s status.

VNCH provides services to patients whose medical condition no longer requires an acute facility stay, but still requires the expertise and knowledge of a skilled nurse or therapist to ensure their health, well being and safety.
